Mandatory Procedure
1. The consolidated annual report must include, as a minimum, the following information for each Individual or Class deviation from the FAR, DFARS or AFFARS:
a. Contractor name and contract number (list only the initial contract for a Class deviation)
b. Description of the contracting action, including dollar amount
c. Unit identifier
d. Annotate Individual or Class deviation
e. Applicable FAR, DFARS, or AFFARS reference
f. Copy of the approved Individual or Class deviation
